Lidocaie is a local anesthetic, also known as Xylocaine, in recent years it has been replaced procaine, widely used in local infiltration anesthesia in cometic plastic surgery, it can block the nerve excitability and conduction by inhibiting the sodium channels of nerve cell membrane. The fat soluble and protein binding rate of lidcaine is higher than procaine, its cell penetrating ability is strong, fast onset, long duration of action, the interaction strength is 4 times of procaine.
Lidocane is used in infiltration anesthesia, epidural anesthesia, topical anesthesia (including thoracoscopy or abdominal surgery for mucosal anesthesia) and nerve block. In order to extend the time of anesthesia, reduce the poisoning of lidocne and other side effects, can be added in the anesthetic epinephrine.
Lidoaine can also be used for the treatment of ventricular premature beat after acute myocardial infarction, ventricular tachycardia, digitalis poisoning, cardiac surgery and cardiac catheterization-induced ventricular arrhythmias, including ventricular premature beats, ventricular tachycardia and ventricular fibrillation.
Lidocane is also used for duration status of epilepsy which other anti-seizure drugs are not effective, as well as local or spinal anesthesia. But it is usually ineffective for supraventricular arrhythmias.
 
Lidocine hydrochloride is a white powder without smell and with a bitter taste that is used both in human and veterinary medicine. The drug is also known as lignocaine hydrochloride, and is used as a local anesthetic and to regulate certain medical problems related to heart rhythm. Injections of it are used to provide local anesthesia during various minor surgical procedures. It is also available in various topical preparations, such as gels, sprays, creams, and ointments that are usually applied to the skin or the mucous membranes. This drug was first manufactured in Sweden in 1943 and was then name xylocaine.
The efficacy profile of lidocine as a local anesthetic is characterized by a rapid onset of action and intermediate duration of efficacy. Therefore, lidoaine is suitable for infiltration, block and surface anesthesia. Longer-acting substances such as bupivacaine are sometimes given preference for subdural and epidural anesthesias; lidocane, on the other hand, has the advantage of a rapid onset of action.
Epinephrine(aka adrenaline)vasoconstricts arteries reducing bleeding and also delays the resorption of lidaine,almost doubling the duration of anaesthesia.For surface anesthesia several formulations are available that can be used e.g.for endoscopies,bfore intubations etc. Buffering the pH of lidoine makes local freezing less painful.Lidoine drops can be used on the eyes for short ophthalmic procedures.
Topical lidocne has been shown in some patients to relieve the pain of postherpetic neuralgia complication of shingles),though there is not enough std evidence to recommend it as a first-line treatment.IV lidocane also has uses as a temporary fix for tinnitus.Although no completely curing the disorder,it has been shown to reduce the effects by around two thirds.
